I, the coach, on behalf of the team, hereby acknowledge and agree that the use of the All Star Field House LLC facilities and equipment (hereafter referred to as Facility) acknowledge that the use of the Facility carries with it the potential for certain risks, including but not limited to bodily injury, death, or property damage.

As the coach of the team, I understand that it is my responsibility to ensure that all team members and their families (hereafter referred to as "Participants") have read, understood, and signed this Liability Waiver before using the Facilities. Furthermore, I attest that all Participants are physically able to participate in the activities offered at the All Star Field House LLC. I also acknowledge that all team Participants and/or guardians of minor Participants have read this waiver and agree.

I hereby release, indemnify, and hold harmless the All Star Field House LLC, its owners, employees, agents, representatives, successors, and assigns from any and all claims, demands, causes of action, damages, judgments, expenses, and liabilities, including attorney's fees, arising from or in connection with the use of the Facilities by the Participants. This includes but is not limited to any injury or damage to person or property resulting from the negligence, gross negligence, or intentional act or omission of any All Star Field House LLC employee or agent.
